Everyone else who's commented has spoiled this movie for you by saying explicitly what it's about. What a shame. I saw an excellent print of Anguish at the movies at the Incredibly Strange Film Festival in Wellington, New Zealand a few years ago, and it was great. It demands being watched in the cinema, preferably with Dolby. You'll know what I mean when you see it. Don't read anything else about it, just see it you probably won't get a chance to see it theatrically so just rent the video . Bigas Luna is a great director see also Reborn and Jamon Jamon .

I first saw this film in a theater. It's the only way to get the full effect which caused me to rate it highly. In the theater you are drawn into the action. The film begins as the story of an klutzy optometric orderly who gets fired from his job because of a patient complaint. When mother finds out she goes into a frenzy exercising her hypnotic control over her son to exact horrible vengeance on all involved. them watch a movie titled The Mommy in a theater. Another movie goer becomes hypnotized from watching the movie and goes on his own killing spree in the theater. Eventually the action in the first movie moves inside another movie theater and you find your self in a theater watching a killer's mayhem in another theater as those in that theater watch another killer's action a third theater. Naturally the killers sneak up on their victims from behind causing you to look over your shoulder every once in a while. You'll lose a lot watching this at home but it's still worthwhile seeing for it's unique style.

I have just watched "Angustia" for the fourth or fifth time, now on DVD, and I do not get tired of this original, bizarre and surrealistic cult-movie, very underrated in IMDb. I am a fan of Bigas Lune, and in my opinion "Angustia" is his masterpiece. The story is simple, but tense, with a soundtrack that recalls Dario Argento's movies. Michael Lerner, in the role of the mad ophthalmologist; Talia Paul, in the role of a scared viewer that becomes impressed with the film she is watching on the screen; and Ángel Jovè, in the role of a lunatic killer, are perfect. Anguish truly is a unique experience. At one point I had to pause because my heart was beating too fast. It was the very effective visual of people struggling with the same thing on screen, had never thought a film could affect me like this. Almost wanted to throw up in parts, and just because of the sound and bizarre visuals, rather than gore. It's not just a horror movie, it's an experiment into film-within-film with audience members watching a movie at a theater about an insane killer and his mother collecting eyes - with the way it's made disturbing and upsetting many of them <more>

I can only imagine how insane it would have been watching this in a movie theater as intended. This has got to be one of the most original concepts for a film I have ever seen! This film takes the 'film within a film' a concept used in other horror films such as Lamberto Bava's "Demons" concept to an entirely new level! "Mother" convinces her son to stalk a movie theater during a screening of "The Lost World" and murder the patrons one by one. She also commands him to remove the victims eyes and bring them to her. She believes that collecting eyes will somehow cure her son's illness and blindness. However, at this point the film takes a strange twist and we now realize that the film we have been watching is actually a separate movie called "The Mommy" that is being viewed by patrons in yet another movie theater who are being stalked by a man who is obsessed with the film. As the patrons of the theater witness the brutal slayings happening on the screen in front of them, they too are murdered one by one by the psychopath that is loose in their own movie theater! Now the viewer realizes that they are actually watching a film within a film within a film! It is now that the film reaches it's bizarre and unusual climax with an unbelievable ending. "Anguish" is not for the squeamish or faint of heart as the eye gouging and murder sequences are pretty intense and gory but it is most certainly one of the most original concepts for a horror film that I have seen in a long time and unfortunately, yes it is currently being remade!

Before the film even begins, a message is displayed on screen warning you about the possibility of hypnosis and dizziness etc. In my opinion, that's a great way to hype someone up. And with hype, disappointment usually follows. But not with Anguish, this was a really cool little flick.The flick actually is pretty much two movies in one. It's a pretty simple premise at first, but once the other story kicks in, the movie's tension and entertainment value is severely upped.The direction and atmosphere is all fantastic, the only minor problems is the acting, it's pretty amateur to a degree. But there are also strong performances as well, the son/murderer did a really good job. And it's killing me trying to recall what I remember him from! And at under 90 minutes, the movie doesn't last too long. Poifect! It's a rare lil horror flick that should definitely have more of a following. With a cool idea leading the way Anguish is one flick any fan of horror or of our beloved lil Zelda should without a doubt check out.
This movie plays with the audience expectations in a level and maybe it have some characteristics of the terror genre that can be seen in a lot of movies but have some features of its own that makes the movie an original one. It's clear that this movie was not made with a big budget but the cast and crew give life to an imaginative screenplay .Rubinstein and Lerner are really great here.
